flucloxacilline iv 4000mg continuous per 24 hours 3 to 5 days

cefazoline iv 6g continuous per 24 hours 3 to 5 days
Remarks:

Bij niet-ernstige penicilline-allergie, opgenomen patient

clindamycine po 600mg 3dd 3 to 5 days
Remarks:

Bij ernstige penicilline-allergie OF niet-ernstige infectie, poliklinische patient

General comments

Geen systemische behandeling tenzij:

  1. er verdenking bestaat op een infectie door Clostridium spp of een ß-haemolytische streptococ van groep A
  2. de wondinfectie gelocaliseerd is in het gelaat of in weefsel met vitale functie(s)
  3. het een immunogecompromitteerde patiënt betreft
  4. er symptomen zijn van een systemische infectie
  5. de wondinfectie gelocaliseerd is (of vermoed) in de nabijheid van een implantaat (osteosynthese, gewrichtsprothese) danwel na het inbrengen daarvan.

NB: Indien een postoperatieve infectie plaatsvindt na gastrointestinale, perineale, urogenitale  of axillaire chirurgie is er mogelijk een indicatie voor bredere (Gram-) dekking. Overleg zo nodig met arts-microbioloog of internist-infectioloog.
